DEEST requires the petition form and the forms required for Graduate Division.
Graduate Division Forms
The Graduate Division requires (paper copy) forms for a student enrolling in a Designated Emphasis program. A copy should also be provided to the DEEST for record keeping.
1. The student should submit the "Change of Major or Degree Goal" petition to the Degrees Unit, 302 Sproul Hall, to indicate admission to the DEEST. Upon receipt of the appropriately signed petition, the addition of the DEEST will be entered into the Graduate Division and Registrar's databases.

Form: Graduate Petition for Change of Major Degree Goal": This is important since the student must be admitted to the DEEST before the qualifying examination.
2. Graduate Division forms
a. Qualifying exam application form: should be signed by DEEST Head Graduate Advisor.

3. When students are advancing to candidacy:
a. Advancement form should also be signed by DEEST Head Graduate Advisor re Dissertation committee.

DEEST Head Graduate Advisor, verifying that all requirements for the DE re coursework have been met prior to filing the dissertation.
4. When the student files his or her dissertation, this will trigger the DE to appear on the Degrees List and be entered into the student's transcript in the Registrar's office as awarded. This form gets sent to the home department of the student. The home department will provide the form to the student, who then has the form signed by the DEEST Head Graduate Advisor, and returns it to the Graduate Division in Sproul Hall. Another form. will then be sent from the Graduate Davison to the home department of the student," Final Report for the Designated emphasis in…" which in turn gets signed by the DEEST Head Graduate Advisor. The original goes to the Graduate Division; a copy is kept by the DEEST.
